Especially Designed for the Employees of Chamber of Commerce Association of AL

The plan pays a lump sum benefit to the insured in addition to any other insurance on the diagnosis of a covered critical illness.

Covered Critical Illnesses are:
Heart Attack, Cancer, Stroke, Organ Transplant, or Kidney Failure

Rates do no change as insured goes into a different age group.
Plan is available for employees between the ages of 18-64.
The plan is guaranteed renewable and portable.
Benefits reduce by 50% when employee reaches age 65.
Child(ren) Rider: Flat $2500 benefit covering ALL children for a flat premium amount of $2.17 per month. Children are covered issue age 15 days to age 19.
